• Resident needs supervision daily
• Advanced medication administration (i.e. includes extra monitoring, treatments, etc.)
• Excessive medical alarm system use (pattern of excessive calls,
i.e. 5 calls a day times 5 days)
• Occasional incontinence cleanup
• Assistance with every transfer
• Assistance to the bathroom
• Treatments and oxygen concentrator assistance
• Advanced dressing needs (i.e. daily dressing to include most clothing, shoes, socks, etc.)
• Assistance with personal hygiene
• Resident needs to be helped in and out of bed
